Your dental implant passport, guarantee and records
An implant passport identifies the device and components used. It is not the same as a treatment guarantee, a clinical record or a promise that another dentist will provide follow-up care.

What an implant passport can record
Manufacturer passport examples show fields for implant type, surface, connection, dimensions, placement date, tooth position, reference or lot number, traceability labels, abutment and restoration details, and the treating professional's stamp.
Placement record
Date, tooth position and the treating clinician or clinic details recorded by the professional responsible for the entry.
Restoration components
Abutment and restoration or coping details, with their own reference and traceability information when the passport provides those fields.

Device-level information matters for traceability. NHS England describes implant data as including the device used, the procedure and clinicians involved, supporting patient safety and device review or recall.
Read a guarantee as a contract term
The word guarantee does not tell you what is covered. Ask for the actual document before treatment and check these points:
Who issues it?
Identify the legal clinic or company responsible. A manufacturer warranty, clinic guarantee and coordinator statement are different things.
What is covered?
Check the exact treatment or component, start date, duration, exclusions, maintenance conditions and evidence required for a claim.
What is the remedy?
Check whether the remedy is assessment, repair, replacement or another defined action, plus who pays for travel, accommodation and local care.

Passport, guarantee and records questions
Is an implant passport a guarantee?
No. A passport records device and treatment identifiers. A guarantee is a separate set of contractual terms issued by the organisation named in the document.
Does a manufacturer warranty cover the whole treatment?
Do not assume so. Ask which component the manufacturer covers, what evidence is required and which clinical, laboratory, travel or follow-up costs are excluded.
Will a UK dentist use records from abroad?
A record pack gives another clinician useful information, but each dentist decides whether they can safely provide a particular service. Arrange local continuity before treatment rather than assuming it.
Can I ask for digital copies?
Ask the treating clinic how records are supplied, in which format, how long they are retained and how to request another copy. Keep your own secure copy after receiving it.
What if the written terms and sales explanation differ?
Pause and ask for the difference to be resolved in the signed document before paying or consenting. Keep the final version you agreed.
